Dear Parents:

As required by New York State Education Department, all students planning on playing a sport must have an Athletic Physical.  The results of the physical shall be valid for a 12 month period.  Before participation in each Sports Season, parents must complete an interval health history, “Sports Candidate Screening Form” (white card).  School nursing personnel must review every student’s health history to ensure that any health problems are identified and considered.

The Arlington School District offers high school students Athletic Physicals during January and June Regents week only.   June physicals are for Fall and Winter Sports.  January physicals are for Spring Sports.  Physicals are done by appointment only and take approximately one hour.  Students must sign up in the Health Office for their physical.  Dates for physical sign ups will be posted in the Athletic Department and announced during morning announcements.

Students should not sign up for a physical on the same day they have a regents exam.

 

Arlington High School requires the initial sports physical to be done by the school physician and her staff. 

 

Sports physicals done by private physicians will be accepted after the school examiner has done the initial one.  However, private physician forms must indicate that the student is cleared to participate in competition sports

 

A “Sports Candidate Screening Form” (white card) is required for each sport.

Note:  10th grade sports physicals done by the school physician and her staff fulfills the New

              York State mandated physical requirement.

MEDICALS-SPORTS PHYSICALS

1.

All participants must have received a physical examination from the school physician within one calendar year prior to tryouts/practice. The school will make arrangements to schedule sports physicals and notify the students when and where the physicals will be administered. The school will offer sports physicals during the final exam week in January and June of each year. It is the responsibility of the student to schedule and keep his/her appointment. If a student does not receive a sports physical when offered by the school, he/she must be given permission from the Athletic Director to receive a physical at the office of the school physician. The physician’s office will charge a fee that must be paid by the student/parents.

* As of 2003, students may receive a sports physical by his/her own doctor.  The student's physician must fill out the forms provided by the school's health office. This policy only applies to students who have obtained his/her initial sports physical by the school physician.

2. All students must have parents/guardian fully complete and sign the white medical update card for each sport the athlete participates in. This card must be returned to the Health Office at least two weeks prior to tryouts for review by the school nurses. The card must be signed and dated less than 30 days prior to the beginning of the season.
3. Clearance notes from the student athlete’s doctor are required for any recent injuries, surgery or chronic illness. If the athlete has seen a specialist (Orthopedist, Cardiologist, etc.) the clearance must be from that specialist.
4. Updated Doctor’s prescriptions are needed each year for inhalers, epi-pens, etc.
